Título: SMOKELESS TOBACCO USE AND ASSOCIATION WITH HEAD AND NECK CANCER AND LEUKOPLAKIA IN PAN-AMERICAN REGION: A META-ANALYSIS

Resumo: Objective: to investigate the prevalence of smokeless tobacco (SLT) use in the Pan-American Health Organization (PAHO) region and the association with head and neck cancer and oral leukoplakia.
Study Design: We performed a search in nine databases (PubMed, Scopus, Embase, Web of Science, SciElo, LILACS, IBECS, BBO-ODONTOLOGIA, and PAHO-IRIS) and grey literature to identify targeted the observational studies. The Joanna Briggs Institute Critical Appraisal Checklist was used to evaluate the risk of bias. Also, we used the GRADE tool to assess the certainty of evidence among the outcomes. A proportion and binary meta-analysis were performed to investigate the overall prevalence and the associations.
Results: Fifty-nine studies were included representing six PAHO countries. The SLT prevalence was 15% (95%CI:11.93-18.69); 17% (95%CI:13.25-22.65) of adults and 11% (95%CI:8.54-14.78) of paediatric patients living in the PAHO region consume SLT. Head and neck cancer (OR= 1.98, 95%CI:1.54-2.55) and oral leukoplakia (OR= 8.38; 95%CI:1.05-67.25) had a positive association with SLT use. Conclusions: The available evidence showed a 15% SLT consumption in the PAHO region with different distribution between the countries. A positive association for head and neck development in adults supports the harmful effects caused by SLT use.
