Resumo: A 66-year-old woman reported "discolored patches and burning mouth" for 6 months. On physical examination, there were violaceous spots on the skin and bilateral white striae on the buccal mucosa. Under the diagnostic hypotheses of Lichen Planus (LP), an incisional biopsy was performed. Microscopically, there was parakeratinized stratified squamous epithelium, band-like lymphocytic infiltrate in the underlying connective tissue, consistent with LP. Three years later, a new incisional biopsy was performed on a white, corrugated plaque on the inferior alveolar ridge. Microscopically, there was keratinized stratified squamous epithelium, with an abrupt keratinization pattern change, atrophy and several mitosis above the basal layer, besides diffuse chronic inflammation in the connective tissue. The microscopic diagnosis was hyperkeratosis with moderate epithelial dysplasia. The final diagnosis was proliferative verrucous leukoplakia (PVL). The diagnosis of LP and PVL is challenging, due to several overlapping features.
