Resumo: Primordial Odontogenic Tumor (POT) is a benign lesion of mixed odontogenic origin that mainly affects the posterior mandible of young patients. This report aims to describe a case of POT in a 12-years-old female patient presenting an asymptomatic left facial asymmetry. In the intraoral examination, swelling in the buccal vestibule of the left maxilla was observed. The computed tomography revealed a hypodense, unilocular, and circumscribed lesion associated with an impacted tooth and extending from the left maxillary sinus to the orbital floor. The enucleation of the lesion with the extraction of the involved tooth was performed. Microscopically, the proliferation of spindle cells in a fibromyxoid mesenchymal tissue surrounded by columnar odontogenic epithelium was observed. The final diagnosis of POT was established, and the patient is under a 28-months follow-up with no signs of recurrence. Despite the indolent behavior, it is important to properly diagnose POT to avoid invasive surgical approaches.
