Resumo: The objective is to report a triple-negative breast cancer metastasis in the oral mucosa. A 36yo female patient was diagnosed with triple-negative left breast invasive ductal carcinoma in 2019, stage IIIA. In June/2021, she was diagnosed with brain, adrenal, nodal and renal metastasis treated with holocranial radiotherapy and CAPITELLO chemotherapy protocol. In September/2021, she presented an exophytic oral mucosa nodule in the right retromolar region, fibrous, purplish-red color in association with fixed and painless lymph node enlargement in the right submandibular region. Clinical hypothesis was peripheral giant cell lesion, metastasis or new primary. Incisional biopsy revealed an epithelioid malignant neoplasm. Immunohistochemistry showed triple negative (ER,PR,HER2), GATA3+ and Ki-67 positive in 80% of the malignant cells. The final diagnosis was breast carcinoma metastasis. The patient underwent radiotherapy in the cervical and mandibular region in association with capecitabine. Last follow-up showed regression of the lesion and the patient is being in follow-up.
