Resumo: Background. Schistosomiasis (blood flukes) is a common parasitic disease globally, yet only one oral lesion has been reported (in periodontitis). We present the second case, the first in a periapical infection. Case description. A 41-year-old male was referred to an endodontist for evaluation and retreatment of a painful mandibular right second premolar. Radiographs showed a routine apical radiolucency with a distal projection reaching the adjacent molar, and with partial destruction of the lingual cortex. Diagnostic Methods, Diagnosis. Apicoectomy with curettage showed granulation tissue in the apical region, but the distolateral radiolucency showed only ischemic marrow damage, with a greatly dilated arteriole (nutrient canal?), which contained a cluster of wormlike structures plugging the vessel lumen completely. Admixed with the "worms" were fluke-shaped organisms and short oval larvae. Ziehl-Neelsen and other staining confirmed a diagnosis of schistosomiasis. Management. An Infectious Disease specialist found multiple systemic shistosomiasis effects, treated successfully via praziquantel.
