Resumo: Oral adverse effects associated with biologic therapies have been increasingly reported, particularly in patients treated with interleukin inhibitors. This case reports a 19-year-old female with a two-year history of psoriatic arthritis who, after switching from ixekizumab to secukinumab, developed oral lesions shortly after the second dose. Clinical examination revealed multiple symptomatic ulcers on the lateral and ventral surfaces of the tongue, floor of the mouth, and marginal gingiva. The lesions showed complete remission within six days following photobiomodulation and topical dexamethasone. However, nine days later, non-scrapable white plaques developed on the tongue, associated with burning sensation. Antimicrobial photodynamic therapy was performed and nystatin oral suspension prescribed, resulting in complete resolution within three days. Findings may suggest a drug-related reaction or herpetic-like lesions followed by chronic hyperplastic candidiasis, potentially associated with drug-induced immunosuppression. This case highlights the importance of recognizing and managing oral manifestations associated with biologic therapies.
