Resumo: Ameloblastoma is a benign but locally aggressive odontogenic neoplasm with high recurrence rates following conservative treatment. Wide surgical resection remains the standard of care, often resulting in significant mandibular defects requiring individualized reconstruction.
We present three patients (ages 14, 35, and 41) with extensive mandibular ameloblastomas. Case 1 involved segmental resection and immediate reconstruction using a custom 3D plate combined with an adipose-derived stem cell autograft. Case 2 utilized a two-phase approach: resection with a space maintainer and umbilical cord stem cell culture, followed by delayed reconstruction with a 3D mesh prosthesis and cell graft. Case 3 underwent segmental resection with immediate fibula free flap reconstruction. Postoperative follow-up confirmed excellent graft integration and restoration of mandibular continuity in all cases.
Individualized surgical planning, incorporating patient-specific implants, stem cell therapy, or microvascular flaps, ensures successful functional outcomes and facilitates subsequent oral rehabilitation with osseointegrated implants.
