Título: EXTENSIVE OSTEOLYTIC LESION MIMICKING ODONTOGENIC CYSTS: A CASE REPORT OF SQUAMOUS ODONTOGENIC TUMOR

Resumo: A 26-year-old male without relevant medical history was referred for preoperative assessment of a cystic lesion associated with an impacted mandibular third molar right (#4.8). Cone-beam computed tomography demonstrated a large osteolytic lesion extending from retromolar region to the distal root apex, with intimate contact with the mandibular canal, marked alveolar bone resorption, and buccolingual cortical expansion without a well-defined corticated boundary. Provisional diagnoses included paradental cyst of Craig, dentigerous cyst and odontogenic keratocyst. Surgical management consisted of extraction of the impacted tooth with complete enucleation of the lesion. Histopathological analysis revealed multiple islands and anastomosing cords of cytologically bland squamous epithelium, exhibiting focal vacuolization and pseudocystic changes within a fibrous connective stroma containing chronic inflammatory infiltrate, establishing the diagnosis of squamous odontogenic tumor. The postoperative course was uneventful, and the patient remains under periodic clinical and radiographic surveillance. This case underscores the diagnostic challenge posed by this rare entity.
