Título: FUTURE PERSPECTIVES OF INVESTIGATOR-INITIATED TRIALS IN ORAL MEDICINE: A SCOPING REVIEW

Resumo: Objective: To map the landscape of investigator-initiated trials (IITs) in Oral Medicine by characterizing the methodological and regulatory framework of the IIT model and identifying the empirical evidence.
Study design: A scoping review conducted according to PRISMA-ScR guidelines. The methodology followed a two-tier approach: 1) A review of the literature focused on the conceptual and regulatory aspects of the IITs, and 2) A systematic review of IITs published between 2021 and 2026 in the highest-impact Oral Medicine journals.
Results: A total of 82 records were included. Twelve studies provided outlining advantages and barriers of independent research. In parallel, 70 published IITs were identified. Among these trials, self-funding predominated (45.7%), followed by government (27.1%) and university funding (21.4%). Asia contributed the largest proportion of trials (35.7%), followed by Europe (25.7%). Regarding clinical innovation, new drug applications were the most frequent (24.3%), followed by photobiomodulation (22.9%). The most studied conditions were oral lichen planus and mucositis (21.4%).
Conclusion: This scoping review identified a robust number of published IITs, while in the first phase the results emphasize that Oral Medicine IITs are strategic but untapped. Strengthening independent research could increase the impact of these trials and generate relevant evidence to guide clinical practice.
