Resumo: This study aims to present a case of atypical pyogenic granuloma in the alveolar ridge, emphasizing the importance of differential diagnosis prior to prosthetic planning and long-term clinical follow-up. A 58-year-old female patient sought care for dental prosthesis fabrication, reporting previous extraction of tooth 46 performed six months earlier. Ten days after the procedure, a lesion with progressive growth was observed. Clinical examination revealed unilateral swelling and an exophytic, sessile, whitish, and bleeding lesion. Radiographically, bone resorption was observed in the right mandibular ramus. An excisional biopsy was performed with a diagnostic hypothesis of pyogenic granuloma, later confirmed by histopathological analysis. After three years of follow-up, the patient showed complete healing with no signs of recurrence. Although it is a common lesion, extensive presentations require accurate diagnosis to rule out more aggressive pathologies. Proper identification was essential for successful prosthetic rehabilitation and long-term clinical stability.
