Título: Association between apical periodontitis and medication-related osteonecrosis of the jaw and osteoradionecrosis

Resumo: Objective: This pioneering study aims to investigate the potential association between apical periodontitis (AP) and the occurrence of medication-related osteonecrosis of the jaw (MRONJ) and osteoradionecrosis (ORN).
Study design: This cross-sectional observational study included participants with MRONJ or ORN and a control group of patients treated with antiresorptives and radiotherapy without osteonecrosis. Participants with bone metastasis in the jaws were excluded. Clinical and imaging data were collected and statistically analyzed (p < 0.05).
Results: A total of 133 patients were included. Seventy patients with osteonecrosis (61.4% MRON and 38.6% osteoradionecrosis) and 63 control patients were included. Most patients were female (51.9%) with other comorbidities (83.5%). The mandible was the most frequently affected site in both conditions. AP within osteonecrotic areas was identified in 23.3% of MRONJ and 29.6% of ORN cases, predominantly affecting molars (p < 0.05). Notably, a significant association was observed between AP and the presence of Actinomyces (p < 0.05).
Conclusion: These findings support a potential clinically relevant association between odontogenic infections and MRONJ and ORN. Furthermore, the importance of early diagnosis and management of infections in at-risk patients supports a more accurate, biologically oriented dental approach in oncologic care.
