Resumo: Diagnostic overlap between medication-related osteonecrosis of the jaw (MRONJ), odontogenic infection, and metastatic disease represents a significant challenge in oncologic patients. To report a case emphasizing paresthesia as a clinical sign that may mimic metastatic disease in the context of MRONJ. A 52-year-old woman with a history of luminal breast cancer and established bone metastases, receiving denosumab, presented with pain and swelling in the region of tooth #38, initially diagnosed as pericoronitis. After temporary improvement with antibiotics, she developed paresthesia in the inferior premolar region despite preserved pulp sensibility. Further evaluation revealed apical periodontitis in a previously treated mandibular molar. Imaging findings and rapid neurological progression, raised strong suspicion of metastatic involvement; however, MRONJ remained a differential diagnosis. A CT-guided biopsy was performed and confirmed MRONJ. Paresthesia may represent a misleading clinical sign, mimicking metastatic disease in MRONJ, reinforcing the need for multidisciplinary evaluation and histopathological confirmation.
