Resumo: A 56-year-old female presented to our service with a large lesion in the anterior mandible of approximately two months duration. CT imaging showed a hypodense lesion with destruction of both vestibular and lingual cortices. The patient also presented osteolytic lesions in other bones. Incisional biopsy revealed a neoplasm composed of a diffuse proliferation of plasma cells, exhibiting cellular atypia and mitotic figures. Focal deposits of amorphous eosinophilic material consistent with amyloid were identified and demonstrated apple-green birefringence under polarized light following Congo red staining. Immunohistochemical analysis revealed positivity for CD138 and EMA, with immunoglobulin monoclonality for kappa light chain. The Ki67 proliferation index was approximately 5% in hotspot areas. These findings led to the diagnosis of multiple myeloma manifestation in the gnathic bone. The patient was referred for oncohematological treatment.
