Resumo: Oral carcinoma cuniculatum (OCC) is an extremely rare variant of squamous cell carcinoma that often poses diagnostic difficulties on biopsy despite clinical indications of malignancy. An 83-year-old female patient presented with a progressively growing ulcer on her right buccal mucosa, present for 4 months, without healing. She had no history of alcoholism or smoking. Clinically, an infiltrative ulcerated lesion was observed on the right buccal mucosa with everted borders, a necrotic bed, and a hardened consistency, measuring approximately 2.5 cm in its largest diameter. Cytologic exam showed atypical cells of undetermined significance. Histopathological examination revealed well-differentiated stratified squamous epithelium, numerous branched crypts filled with keratin exhibiting a burrowing pattern similar to "rabbit holes", leading to a diagnosis of Carcinoma Cuniculatum. This case expands knowledge about the morphological characteristics of this rare variation of oral carcinoma.
