Título: Chronic intrasinusal actinomycosis mimicking salivary gland neoplasia in cystic inflammatory odontogenic lesion

Resumo: 62-year-old female patient presented a soft tumor on the palate with bone destruction and projection to vestibular mucosa. Radiographic examination revealed invasion of the left maxillary sinus. Patient reported an abscess in the region approximately 10 years ago. FNAC yielded a mixed-appearance fluid, partly bloody, partly yellowish semi-solid, with a mucus-like consistency and a fetid odor. During the biopsy surgery, a surgical window was opened in the hard palate, revealing a hollow space with buccosinusal communication, draining serosanguineous content associated with sulfuric granules. Debridement of the left maxillary sinus was performed, and histopathological analysis confirmed the diagnosis of a radicular cyst secondarily infected by actinomycosis and chronic sinusitis. The extraction of tooth 25 was performed the following week. After this, good clinical progress was observed, with adequate healing, complete regression of the lesion, and absence of signs of infection or recurrence. The patient continues with periodic follow-up.
