Resumo: Objective: To describe oral medicine e-consultations at the Chilean Ministry of Healths Digital Hospital (July 2020-July 2025), assessing demographics, priority levels, response times, diagnostic agreement, and specific diagnoses.
Methods: This cross-sectional study analyzed 26,136 e-consultations. General dental practitioners provided clinical images and preliminary diagnoses for specialists, who evaluated the cases via the Digital Hospital platform. A subsample of 515 cases was analyzed for specific diagnostic frequencies.
Results: Consultations spanned 15 regions and 1,003 primary health centers. Most patients were women (63.7%) and 38.2% were aged ≥60. The median response time was 45 hours, with 63.4% diagnostic agreement and a 19.5% primary care resolution rate. Priority levels were: 21.2% high, 37.1% medium, and 41.7% low. Subsample analysis identified leukoplakia (5.6%), oral lichenoid disease (1.9%), actinic cheilitis (1%), and squamous cell carcinoma (2.1%).
Conclusions: The program facilitates rapid specialist assessment of oral mucosal lesions, supporting primary care management. Approximately 11% of cases involved suspected potentially malignant disorders or squamous cell carcinoma, highlighting the systems role in early detection of oral cancer within the public health network.
