Título: PIEZOELECTRIC OSTEOTOMY AND TEMPORALIS INTERPOSITION FOR TEMPOROMANDIBULAR JOINT ANKYLOSIS: A CASE REPORT

Resumo: Temporomandibular joint (TMJ) ankylosis is characterized by pathological fusion between the mandibular condyle and the glenoid fossa, resulting in bony synostosis. A thirty-year-old female patient presented with severe limitation of mouth opening, impairing oral hygiene and mastication. Reported mandibular trauma at age 6 and onset of progressive functional restriction 10 years ago. Clinical examination showed mouth opening of less than 23 mm, mandibular deviation to the left, and pain during maximal opening. Computed tomography revealed absence of joint space, as well as erosion and bone degeneration of the left TMJ, consistent with bony ankylosis. Treatment consisted of arthroplasty under general anesthesia, with removal of the ankylosed block using piezoelectric ultrasound and interposition of an autogenous temporalis myofascial flap. The procedure was uneventful, with satisfactory mandibular mobility. During follow-up, improvement in mouth opening and mastication, as well as preservation of physiological functions, were observed, associated with physiotherapy to prevent recurrence.
