Resumo: Osteolipoma is an extremely rare histological variant of lipoma, particularly in the head and neck region. This study reports the case of a 54-year-old Caucasian male patient who presented with an asymptomatic swelling in the left anterior region of the mandible, with a three-year history of progression. Intraoral examination revealed a sessile nodule in the oral vestibule with a hard consistency. Computed tomography (CT) showed a hypodense lesion interspersed with hyperdense foci. Surgical excision of the lesion was performed, and the specimen was submitted for histopathological analysis. Histological sections revealed mature bone tissue interspersed with adipose tissue, leading to a final diagnosis of osteolipoma. Although the surgery was uneventful, the patient developed postoperative paresthesia of the mental nerve. This was managed with photobiomodulation (PBM), resulting in complete recovery after three weeks. The patient is currently under clinical follow-up, with no signs of recurrence.
