Resumo: Actinomycosis is a chronic bacterial infection rarely associated with dental implant failure. This case series describes six patients (mean age 66.8 years; female-to-male ratio 5:1) diagnosed with peri-implant actinomycosis between 2010 and 2020. The mandible and maxilla were the most commonly affected sites. Four patients had a history of bisphosphonate use, and one had diabetes. Radiographic findings included localized bone destruction with ill-defined radiolucent areas. Histopathological examination revealed granulomatous inflammation with sulfur granules and filamentous bacterial colonies, accompanied by osteomyelitis and reactive bone formation. The diagnosis was confirmed through histochemical staining (PAS, GMS, Warthin-Starry) and molecular analysis identifying Actinomyces species. Patients were managed with benzathine benzylpenicillin and amoxicillin. Given its indolent and often asymptomatic course, peri-implant actinomycosis represents a diagnostic challenge. Clinicians should maintain a high index of suspicion for this infection as a potential cause of implant failure.
