Resumo: Objectives: To investigate the connection among the extent of fibrosis, demographic characteristics, clinical features, and epithelial dysplasia in OSF, exploring multifaceted correlations to enhance understanding of disease progression and potential malignant transformation.

Methods: This cross-sectional retrospective study involved 50 patients diagnosed with OSF. New histological sections were prepared from archived paraffin-embedded tissue blocks and stained with haematoxylin and eosin. The sections were evaluated for histopathological characteristics, quantification of fibrosis extent, and assessment of epithelial dysplasia using WHO three-tier and binary grading systems.

Results: A total of 50 OSF patients (mean age 40.34 years, 84% male) with younger patients (<40 years) had a higher rate of epithelial dysplasia (p=0.016). All patients with restricted tongue movement exhibited advanced fibrosis (>1.6mm). 11 OSF patients exhibited no dysplasia, 39 patients presented with varying degrees of epithelial dysplasia. Fibrosis extent was greater in dysplastic cases, with a significant association between increased fibrosis and higher dysplasia grades (p=0.023) across both three-tier and binary grading systems.

Conclusion: This study highlights the association between the extent of fibrosis and increased risk of developing epithelial dysplasia in OSF, further emphasising the extent of fibrosis as an important prognostic marker for evaluating the risk of malignant transformation.
