Resumo: This case describes a 9-year-old female patient who presented with a painless swelling in the left mandible, with a three-month history of progression. Clinically, an expansive and firm swelling was observed in the body of the left mandible. Initial radiographic examination revealed a mixed unilocular lesion with well-defined borders in the left mandible, associated with displacement of tooth germs. The diagnosis of juvenile ossifying fibroma was established through incisional biopsy and histopathological analysis. Considering the clinical and radiographic findings, the lesion was surgically excised in the operating room, with preservation of the cortical bone. During surgery, independent mobility of bone segments was noted, suggesting a pathological fracture, which was managed conservatively without fixation hardware. The patient had an uneventful postoperative recovery, and three years of clinical and radiographic follow-up showed no evidence of recurrence.
