Título: Combined Sclerotherapy and Surgery for Upper Lip Vascular Malformation

Resumo: A 46-year-old previously healthy woman presented with a 25-year history of progressive enlargement of the left upper lip, with no reported history of local trauma. On clinical examination, two nodular lesions were observed on the left upper lip, adjacent to the midline. The lesions were sessile, firm on palpation, and exhibited a violaceous hue. Based on the clinical findings, a provisional diagnosis of a low-flow vascular malformation was established. Initial management consisted of sclerotherapy with Ethamolin® (monoethanolamine oleate); however, no satisfactory clinical response was achieved. An alternative approach using intralesional bleomycin was subsequently employed, resulting in partial lesion regression. In view of this partial response, a combined therapeutic approach was undertaken, integrating sclerotherapy with surgical excision. At postoperative follow-up, the patient demonstrated satisfactory aesthetic and functional outcomes, with no evidence of complications or functional deficits.
