Resumo: Objective: To identify factors that influence the clinical behaviour and disease progression in oral lichenoid diseases (OLD).
Study design: This retrospective cross-sectional study included 63 patients diagnosed with oral lichen planus (OLP) or oral lichenoid reaction (OLR). Patients were categorized based on clinical behaviour (asymptomatic, symptomatic, and recalcitrant groups) and disease progression (maintained or progressed). Sociodemographic and clinical variables were analyzed.
Results: 63.5% of patients were diagnosed with OLP, and 36.5% had OLR. Most were female (81.0%) and Chinese (55.7%), with a mean age of 56.5 years. Clinical behaviour was significantly associated with the presence of desquamative gingivitis (p = 0.039), clinical presentation at biopsy (p < 0.001), and thyroid disease (p = 0.017). Disease progression occurred in 33.3% of patients and was significantly associated with ethnicity (p = 0.027), hypertension (p = 0.028), diabetes (p = 0.016), the use of antidiabetics (p = 0.018), asthma (p = 0.013), and clinical presentation at biopsy (p = 0.013). In multivariable analysis, only asthma remained an independent predictor of disease progression.
Conclusion: These findings highlight the multifactorial nature of OLD and support the need for comprehensive, individualized clinical assessment and management of OLD.
