Resumo: An 84-year-old patient with xanthoderma was admitted in early September 2025 for evaluation of diffuse abdominal pain and dyspepsia. Imaging studies revealed soft tissue masses in the thorax and abdomen, along with enlarged lymph nodes in the inguinal and axillary chains. Lymph node biopsy confirmed high-grade B-cell lymphoma. During the diagnostic workup, the patient reported discomfort and paresthesia in the right mental region, as well as difficulty chewing. Intraoral examination identified a 10 mm nodular lesion on the gingiva in the region of tooth 44, previously extracted, with associated anterior expansion of the adjacent cortical bone. Facial computed tomography demonstrated a hypodense lesion in the right mandibular body containing soft tissue components. The mandibular lesion was surgically excised prior to chemotherapy, and histopathological analysis revealed fibroblastic osteosarcoma. Treatment with R-CHOP was initiated, totaling six biweekly cycles, resulting in complete remission by January 2026.
