Resumo: An 88-year-old female patient presented to a stomatology clinic with a progressive swelling on the left floor of the mouth, evolving over approximately one year. Intraoral examination revealed a sessile nodular lesion measuring approximately 30 mm, with fibrous consistency, smooth surface, mucosa-colored, and painless on palpation. Laboratory tests and a facial CT scan were requested, with sialolithiasis as the primary diagnostic hypothesis. The patient underwent fine-needle aspiration (FNA), yielding blackened contents; removal of an extensive sialolith in the sublingual caruncle region; and an incisional biopsy of the left sublingual gland. Histopathological analysis confirmed the diagnosis of sialolithiasis associated with an incidental finding of low-grade mucoepidermoid carcinoma, without angiolymphatic invasion or perineural infiltration. The patient was counseled and referred for oncologic treatment, also remaining under stomatology follow-up. This case highlights the importance of histopathological investigation in clinically benign-appearing lesions, reinforcing the need for a meticulous diagnostic approach.
