Resumo: Extramedullary plasmacytoma is an uncommon entity, most frequently involving the upper aerodigestive tract (20-30% of cases), whereas primary nodal involvement accounts for <2% of extramedullary presentations. A 79-year-old female presented with a painless, mobile cervical mass at level IIA, with a two-month history, clinically suspected to represent a pleomorphic adenoma. Histopathological examination revealed an atypical lymphoid proliferation with predominant plasmacytic differentiation. Immunohistochemistry demonstrated negativity for CD20, PAX5, and LMP-1, with strong CD138 expression, a kappa:lambda ratio of 6:1, and a Ki-67 index of 40%. A provisional diagnosis of nodal extramedullary plasmacytoma was established. The patient was referred for hematologic evaluation to exclude multiple myeloma; the skeletal survey is currently negative, with low-level M-protein detected. She remains under close follow-up. This case underscores the diagnostic complexity of plasmacytic proliferations in lymph nodes. Primary nodal involvement is unusual and requires careful clinicopathologic correlation and systemic evaluation to exclude plasma cell myeloma.
