Resumo: Objective: To assess the prevalence of chronic apical periodontitis (CAP) in patients with head and neck cancer (HNC) compared to non-cancer patients. Methods: The oncologic group comprised adults (>18 years) diagnosed with HNC, who had preoperative panoramic radiographs and no tumors in other body regions. The control group consisted of individuals with no evidence of malignant neoplasms in the head, neck, or other sites. Groups were matched by sex, age, and number of teeth present (±2 teeth). Apical periodontitis was assessed by two blinded examiners using the Periapical Index (PAI). The Shapiro-Wilk test was used to analyze the distribution, followed by the Mann-Whitney U test to assess the difference between the groups. A value of P ≤ 0.05 was considered significant. Results: The prevalence of CAP was significantly higher in HNC patients (P < .0001) than in controls. The frequency of periapical lesions scored as 4 or 5 on the PAI was also greater in the oncologic group (P < .0003 and P < .0004, respectively). Conclusion: HNC patients demonstrated significantly higher prevalence of CAP compared with matched non-cancer controls. They also exhibited more teeth with large periapical lesions, underscoring the need for thorough endodontic assessment in oncology care.
