Resumo: This case report describes a 15-year-old male with ameloblastoma, a benign yet locally aggressive odontogenic tumor with a recognized potential for recurrence. The patient presented with mandibular swelling associated with a large lesion causing marked cortical expansion. Imaging revealed an extensive multilocular radiolucency on radiography, associated with root resorption of adjacent teeth, and a predominantly solid component on computed tomography, involving the entire mandibular ramus and a substantial portion of the mandibular body. Histopathological examination demonstrated a follicular pattern, confirming the diagnosis. A conservative surgical approach, consisting of curettage, peripheral ostectomy, and extraction of the involved teeth was performed avoiding more aggressive resection. Over a 44-month postoperative period the patient exhibited satisfactory healing with no evidence of recurrence, remaining under close clinical monitoring. This case supports conservative management in selected young patients, including those with extensive lesions, emphasizing its potential to preserve anatomical structures and reduce morbidity without compromising outcomes.
