Título: Extensive diffuse large B-cell lymphoma in the nasomaxillary region: a case report

Resumo: Diffuse large B-cell lymphoma is the most common subtype of non-Hodgkin lymphoma, accounting for approximately 30-35% of cases worldwide. It is an aggressive malignancy that may involve extranodal sites, including the oral cavity and jaw bones, although primary oral lymphomas are rare. A 57-year-old female patient presented with a two-month history of persistent toothache and facial swelling. Clinical examination revealed a firm, non-tender bilateral maxillary swelling. Intraorally, there was expansion of the maxillary vestibular sulcus and mobility of the involved teeth. Imaging studies demonstrated an expansile, infiltrative lesion in the nasomaxillary region. Histopathological analysis revealed a lymphoid neoplasm composed of medium to large atypical lymphocytes. Immunohistochemistry showed CD20 positivity in neoplastic cells and CD3 positivity in peripheral lymphocytes. These findings confirmed diffuse large B-cell lymphoma. The patient is currently undergoing onco-hematological treatment. Lymphoproliferative malignancies should be considered in the differential diagnosis of rapidly growing oral swellings.
