Resumo: A 63-year-old female patient was referred to our service for evaluation of an extensive maxillary lesion. The patient reported a three-month history of progressive facial swelling. She initially sought medical attention and was diagnosed with sinusitis. Extraoral physical examination revealed extensive swelling in the right maxillary sinus region. Intraoral examination revealed swelling in the right alveolar ridge extending to the hard palate. CT exam revealed an expansive and infiltrative lesion affecting the maxillary sinus, alveolar ridge, and orbital floor on the right side. The clinical diagnosis was osteosarcoma. A biopsy was performed, and histopathological examination revealed a spindle cell neoplasm. The IHC analysis revealed positivity for SATB2, KI67 and AML suggesting a diagnosis of osteoblastic osteosarcoma. This case underscores the importance of monitoring the persistence of signs and symptoms and highlights the necessity of re-evaluating the diagnosis when there is a lack of response to initial therapy.
