Título: Basosquamous Carcinoma of the Tongue: A Rare Biphasic Malignancy Challenging Diagnostic Boundaries

Resumo: Basosquamous carcinoma is a rare and aggressive malignant neoplasm with both squamous and basaloid differentiation. We report the case of a 70-year-old male, a smoker and alcohol user, presenting with a painful ulcerated nodular lesion on the dorsum of the tongue with irregular borders, measuring approximately 1.5 cm. Clinical hypothesis included processes and malignant neoplasms. Incisional biopsy revealed infiltrative proliferation of atypical epithelial cells in a biphasic pattern, with squamous and basaloid cells, nuclear pleomorphism, dyskeratosis, frequent mitoses, comedo-like necrosis, and muscular and perineural invasion. Immunohistochemistry was positive for Pan-CK, p40, p63, and Ber-EP4, with a Ki-67 index of 40%. The patient underwent partial glossectomy with free margins, ipsilateral neck dissection, and adjuvant radiotherapy. He has been under follow-up for four years with no signs of recurrence. This diagnosis highlights the importance of clinicopathological correlation and the immunohistochemical panel in differentiating malignant epithelial neoplasms from other epithelial malignancies.
