Título: Polymorphous adenocarcinoma of the hard palate: a diagnostic challenge in oral pathology

Resumo: A 67-year-old female presented with a slowly enlarging, asymptomatic palatal mass of approximately one year of evolution, causing functional discomfort during speech. Clinical examination revealed a lobulated, non-ulcerated lesion of the right hard palate measuring approximately 4 cm. Initial incisional biopsy demonstrated a low-grade monomorphic tubular neoplasm, raising differential diagnoses including pleomorphic adenoma and adenoid cystic carcinoma.
The patient underwent surgical excision. Histopathological evaluation of the resection specimen confirmed a polymorphous adenocarcinoma of minor salivary gland origin, with tubular architecture, low proliferative index (Ki-67 ~25%), and positive immunoexpression for CK7 and p63. The deep surgical margin was involved.
This case illustrates a low-grade malignant neoplasm clinically and histologically mimicking a benign tumor, highlighting the diagnostic challenge and the importance of thorough clinicopathological correlation. Adjuvant radiotherapy was indicated.
