Resumo: Congenital imperforate submandibular duct (CISD) is an uncommon condition characterized by swelling in the sublingual region. This study reports a series of four cases of CISD, with ages ranging from 16 days to 1 year and 5 months (mean age 6 months). The chief complaint was pain or discomfort during breastfeeding, as reported by the mothers. Clinical examination revealed a nodular lesion with a yellowish color or translucent appearance on the sublingual caruncle. Clinical diagnostic hypotheses included mucocele and dermoid cyst. Treatment consisted of complete excision of the lesion, with pediatric dental support. Microscopically, an extensive cystic area was observed, lined partly by stratified squamous epithelium and partly by columnar epithelium. At the periphery, dense fibrous connective tissue, acinar destruction, and ductal ectasia were identified. Follow-up ranged from 4 months to 3 years, with no complications. This study highlights the importance of early diagnosis for the well-being of pediatric patients.
