Resumo: Case report of an extensive periapical lesion associated with endodontic failure in maxillary anterior teeth. A 28-year-old female patient presented with loss of pulp vitality in teeth 21 and 22, with an extensive periapical lesion radiographically evidenced. Four years after initial endodontic treatment, she returned with painful symptoms, facial edema, and persistent lesion. Incisional biopsy was performed with a diagnosis of radicular cyst. Management consisted of cystic decompression with drain placement for 15 months to achieve gradual lesion size reduction. Following the decompression period, endodontic retreatment of tooth 21 was performed, followed by endodontic surgery with lesion enucleation, apicoectomy, root curettage, and bone grafting with biomaterial. Six-month follow-up demonstrated preservation of adjacent bone structures and satisfactory bone healing. Cystic decompression as an initial decompressive treatment, combined with subsequent endodontic surgery, proved effective for managing extensive endodontic periapical lesions.
