Resumo: Idiopathic gingival papilokeratosis with crypt formation (IGPCF) is a rare condition, recently described as white plaque lesions on attached gingiva of unknown etiology. We report three cases in young Black male patients presenting asymptomatic white plaques on maxillary and mandibular gingiva. The lesions showed a granular surface and they were asymptomatic. The patients denied systemic disease, use of medications, or allergies. All patients underwent an incisional biopsy. Histopathological examination revealed gingival mucosa with a thin parakeratin layer, papillary acanthosis, elongated and tapered rete ridges, and multifocal crypt-like epithelial invaginations frequently associated with melanin pigmentation, confirming the diagnosis of IGPCF. One patient underwent gingivoplasty due to esthetic concerns, with satisfactory outcome. The description of these cases expands the clinicopathological spectrum of IGPCF, emphasizing the importance of clinical and histological recognition for differential diagnosis and appropriate therapeutic management.
