Título: SINONASAL TRACT ANGIOFIBROMA: A SYSTEMATIC REVIEW

Resumo: Objective: To synthesize the clinical, imaging, and treatment characteristics of sinonasal tract angiofibroma reported in the last decade. Study design: A systematic review conducted according to PRISMA guidelines and registered in PROSPERO. A comprehensive search was performed in MEDLINE, Embase, Web of Science, Scopus, and LILACS (2015-2025). Observational studies reporting individual patient data were included. Results: Twenty-five studies comprising 25 cases were included. Most patients were male (92%), with a mean age of 31.2 years. The most frequent symptoms were epistaxis (72%) and nasal obstruction (52%). Tumors originated predominantly in the posterior nasal cavity/nasopharynx, with extension to deep facial spaces (60%), paranasal sinuses (48%), and the skull base (44%). Treatment was divided into endoscopic and open surgical approaches, with endoscopic resection and surgical excision being the most common (28% each). Residual tumor was identified in 12% of cases, while recurrence occurred in 16%. Follow-up was ≤1 year in 48% of cases. Immunohistochemical analysis showed limited β-catenin positivity (16%), with one case also positive for AR. Conclusion Sinonasal tract angiofibroma is a rare, locally aggressive entity with variable clinical presentation, underscoring the need for further studies to enhance diagnostic accuracy and guide therapeutic decision-making.
