Resumo: Female patient, 60 years old, melanoderma, presented with an asymptomatic swelling in the anterior region of the mandible, with a 4-month evolution, denying comorbidities or deleterious habits. On clinical examination, a sessile, hardened nodule was observed, with coloration similar to the surrounding mucosa, measuring 15 × 10 mm in diameter, located in the anteroinferior vestibular fornix. Radiographic examination revealed a mixed, multilocular lesion with ill-defined borders, associated with the right mandibular anterior teeth. An excisional biopsy was performed. Histopathological analysis showed an odontogenic neoplasm with proliferation of odontogenic epithelium in small islands, with keratin formation within them, as well as abundant dentinoid material, clusters of ghost cells, and areas compatible with tubular dentin and enamel matrix. These findings confirmed the diagnosis of a dentinogenic ghost cell tumor associated with an odontoma. The patient has been under follow-up for 6 months, with no signs of recurrence.
