Resumo: This report describes an uncommon case of ameloblastoma in the anterior mandible. A 46-year-old male presented with a painless swelling evolving over six months. Clinical examination revealed anterior mandibular expansion without inflammatory signs. Computed tomography demonstrated a multilocular expansive lesion with cortical thinning and focal discontinuity, involving the lower incisors and canines. Incisional biopsy was performed, and histopathological analysis showed odontogenic epithelial islands with peripheral palisading and reverse nuclear polarization, confirming ameloblastoma. Surgical management consisted of segmental resection with 1 cm safety margins beyond radiographic limits. Immediate reconstruction was performed using a titanium plate and tricortical iliac crest bone graft, shaped to restore mandibular contour and enable future prosthetic rehabilitation. The postoperative course was uneventful, with satisfactory graft integration, preservation of mandibular continuity, and functional recovery. After 12 months of follow-up, no recurrence was observed, reinforcing the effectiveness of the proposed approach.
