Título: Osteochondroma of the Midline Hard Palate in an Elderly Patient: A Rare Presentation

Resumo: A 75-year-old woman presented with a 3-month history of an asymptomatic, sessile, dome-shaped swelling at the midline of the hard palate. The 4 cm lesion exhibited normal to yellowish coloration and a smooth surface with superficial vessels. Cone-beam CT revealed a well-defined hypodense lesion centered at the palatal midline, containing irregular hyperdense foci, with destruction of the superior cortical plate and extension into the nasal cavity. Based on a provisional diagnosis of pleomorphic adenoma, an incisional biopsy was performed. Histopathological analysis showed bland fibromyxoid tissue with chondroid differentiation and osseous trabeculae containing marrow elements, some with granular basophilic calcifications. No mitotic activity or cellular pleomorphism was observed. These findings supported a diagnosis of osteochondroma. The patient underwent conservative surgical curettage and remains disease-free after 1 year. This case highlights an exceptionally rare presentation of osteochondroma in the midline hard palate of an elderly patient, posing a significant diagnostic challenge.
