Resumo: Objective: To investigate the relationship of patient interval (PI) and rurality with stage at diagnosis in patients with head and neck squamous cell carcinoma (HNSCC).
Study design: Cross-sectional study including 112 HNSCC survivors using Poisson regression with robust variance. The outcome was stage at diagnosis, categorized as early (stages I/II) or advanced (stages III/IV). The main independent variable was PI, which was self-reported and defined as the time between the first perceived sign or symptom and the first healthcare consultation. Rurality, classified according to area of residence (urban/rural), was examined as a moderator by including an interaction term between PI and rurality.
Results: The median PI was 150 days (IQR: 90-333). Patients with advanced-stage disease had a longer PI than those with early-stage disease (median: 180 vs. 90 days; p=0.041). Compared with patients living in urban areas with a PI <150 days, a higher prevalence of advanced-stage diagnosis was observed among those living in rural areas (PR=1.60; 95% CI, 1.11-2.32), those in urban areas with a PI ≥150 days (PR=1.44; 95% CI, 1.06-1.94), and those in rural areas with a PI ≥150 days (PR=1.82; 95% CI, 1.32-2.50).
Conclusion: Rurality modified the association between PI and stage at diagnosis in HNSCC.
