Título: Distinct TLR4 and TLR7 Expression Patterns in Biopsies from Patients with Clinically Diagnosed PVL

Resumo: Abstract
Objective: To evaluate TLR4 and TLR7 expression patterns in biopsies from patients with clinically diagnosed proliferative verrucous leukoplakia (PVL).
Methods: Immunohistochemistry for TLR4 and TLR7 was performed on 27 biopsies from 16 patients. In this retrospective cohort, biopsies were grouped into pragmatic histologic categories: hyperkeratosis/lichenoid lesions, verrucopapillary hyperkeratosis/verrucous hyperplasia, dysplasia, and squamous cell carcinoma/atypical epithelial proliferation. Staining was assessed separately in the epithelium/tumor and inflammatory infiltrate using H-scores.
Results: TLR4 showed predominantly epithelial staining with variable inflammatory-compartment positivity, whereas TLR7 was absent in epithelium and localized to stromal inflammatory cells. Inflammatory TLR7 expression increased with histologic severity and showed the strongest severity-associated pattern. Inflammatory TLR4 expression also increased with severity, whereas epithelial TLR4 expression was associated with age rather than diagnostic category.
Conclusion: These findings support differential innate immune signaling patterns in biopsies from patients with clinically diagnosed PVL and highlight the importance of the inflammatory microenvironment in disease progression.
