Título: QuPath as a Useful Tool in the Histopathological and Immunohistochemical Analysis of Oral Lichen Planus Samples

Resumo: Objective: to implement the QuPath software for the histopathological and immunohistochemical analysis of oral lichen planus samples, focusing on lymphocyte counting and the expression of markers associated with carcinogenesis in the subepithelial stroma.
Study Design: a descriptive observational study was conducted using 13 samples from patients diagnosed with oral lichen planus, obtained from the Oral Pathology and Medicine Diagnostic Center of the University of Valparaíso, Chile, and from private practice.
Results: the first stage consisted of slide digitization (39 slides in total) and tissue detection through the training and application of a Pixel Classifier. This was followed by tissue segmentation, annotation preparation for cell detection, and stromal stratification. Finally, once the software was trained, immunohistochemical analysis of Bcl-2 and α-SMA was performed. Data were exported to Excel.
Conclusion: the use of digital pathology through QuPath enabled objective and reproducible quantification, reducing interobserver variability and providing spatial analysis by tissue strata. This software represents a highly useful tool for diagnosis, teaching, and research in oral pathology. According to our literature search, this is the first study to use QuPath in the histochemical and immunohistochemical analysis of oral lichen planus.
