Resumo: Objective: To compare the clinical features and demographic profile of patients with medication-related osteonecrosis of the jaws (MRONJ) and osteoradionecrosis (ORN). Study design: Cross-sectional observational study with retrospective analysis of medical records of patients treated at a referral center between 2008 and 2025. A total of 50 cases with confirmed diagnosis of osteonecrosis were included. Descriptive and inferential analyses were performed using Pearson's chi-square test (α=5%) and odds ratios (OR) with 95% confidence intervals. The study was conducted in accordance with an institutional research project and approved by the Research Ethics Committee of the State University of Feira de Santana (protocol no. 087/2008; CAEE 0086.059.000-08). Results: MRONJ was more prevalent (62%) than ORN (38%). A significant association was observed between diagnosis and sex (p<0.001), with higher odds of MRONJ in women (OR=13.46), as well as between diagnosis and age >65 years (p=0.041; OR=3.44). The mandible was the most affected site in both conditions. Smoking showed a trend toward association with ORN (p=0.052). Conclusion: MRONJ and ORN show distinct clinical and epidemiological profiles, particularly regarding sex and age. These findings reinforce the need for preventive strategies and individualized dental follow-up based on the specific risk profile of each condition.
