Título: ORAL SYPHILIS: A CASE SERIES ILLUSTRATING DIAGNOSTIC CHALLENGES AND CLINICAL DIVERSITY

Resumo: Syphilis is an infectious disease occasioned by Treponema pallidum. Oral manifestations may appear at different stages, mimicking a wide range of clinical conditions and creating diagnostic challenges. Here, we report a case series of oral syphilis, based on diverse clinical presentations and histopathological characteristics. Five cases were presented, three female and two male patients aged between 26 and 49, exhibited oral lesions with diverse clinical presentations including whitish plaque-like lesions with ulcerated areas on the tongue, hard palate and lower vestibule, as well as normochromic, papillary nodules on the dorsum of the tongue. The clinical hypotheses ranged from immune-mediated lesions, leukoplakia, erythroleukoplakia, squamous cell carcinoma, condyloma, granular cell tumor, and secondary syphilis. Histopathology demonstrated predominantly plasma cell inflammatory infiltrate in the connective tissue and positivity for anti-Treponema pallidum on immunohistochemistry, highlighting spirochetes in the surface epithelium and confirming the diagnosis. The patients were referred for treatment and are under follow-up.
