Resumo: A 68-year-old woman with rheumatoid arthritis (RA), receiving tocilizumab and methotrexate, was referred for stomatological evaluation due to recurrent oral ulcerations persisting for one year. The lesions, characterized by a fibrinous base and erythematous halo, were refractory to previous treatments, including topical agents and photobiomodulation. An incisional biopsy confirmed mucous membrane pemphigoid (MMP). Initial management with topical dexamethasone failed to achieve sustained remission. After six months, the patient returned with persistent MMP lesions showing a clinically altered, contaminated appearance, along with new punctate ulcerations on the palate suggestive of secondary herpetic infection. Concomitantly, ocular involvement was suspected, prompting referral for ophthalmologic evaluation. Examination revealed corneal stromal infiltrate, limbal epithelial loss, and scleral vasodilation, consistent with peripheral ulcerative keratitis secondary to RA reactivation. The patient was subsequently referred to a rheumatologist for therapeutic adjustment and achieved clinical stability, highlighting the importance of early diagnosis and multidisciplinary management for improved outcomes.
