Resumo: Objective: To analyze the association between salivary dynamics, Candida spp. colonization (saliva/biofilm), and dental caries in HIV+ patients under ART. Methods: A cross-sectional, correlational study of 30 patients at CAPEI-UCV was conducted. Participants primarily utilized ART. Saliva was cultured on Sabouraud Dextrose Agar; CFU/ml utilized a 1,000x dilution factor. PAS staining identified pseudohyphae, while Gram staining differentiated filamentous bacilli in the biofilm matrix, and CPO-D indices were evaluated. Results: Despite 96.6% undetectable viral loads and mean CD4+ counts of 707 cells/dL, caries prevalence was 100% (CPO-D: 18.7 ± 6.21; tooth loss 6.03). Salivary flow and pH (7.1) remained normal due to the substitution of protease inhibitors. Candida spp. colonized 60% of biofilms (77.8% abundant) and 36.7% of saliva; pseudohyphae were present in all positive cases. Biofilm colonization significantly associated with cervical caries severity (p=0.017), HIV chronicity (p=0.006), and ART duration (p=0.041). Salivary colonization correlated with lower CD4 percentage (p=0.027). Actinomyces spp. (45% of sub-sample) correlated with prolonged treatment. Conclusions: Caries in stable HIV+ patients is a chronic dysbiosis where biofilm Candida acts as a critical local determinant of severity. Localized prevention must target fungal-bacterial synergy and the softening of root cementum, as systemic stability does not mitigate oral microbiological shifts
