Título: VALIDATION OF AUTOMATED IMAGE ANALYSIS FOR HER2 GENE AMPLIFICATION IN BREAST AND SALIVARY GLAND DUCTAL CARCINOMAS

Resumo: Objective: To validate an automated image analysis (AIA) method for assessing HER2 gene amplification in invasive breast carcinomas (IBC) and salivary duct carcinomas (SDC) using fluorescent in situ hybridization (FISH) and immunohistochemistry (IHC).
Study design: This multicenter retrospective study included 45 patients (30 IBC, mean age 47 years; 15 SDC, mean age 58 years). Cases were selected based on equivocal HER2 IHC expression (score 2+) with heterogeneous staining and sufficient material for FISH. HER2 amplification was evaluated by manual scoring of at least 120 nuclei by two blinded pathologists and by an automated spectral imaging system. A tissue matching tool was employed to correlate hotspot areas between IHC and FISH sections.
Results: The AIA tool identified HER2 amplification in 43.3% of IBC and 40.0% of SDC, while manual analysis identified 40.0% and 26.7%, respectively. Discordant cases (3.3% IBC; 6.6% SDC) were all reclassified as amplified after consensus review, favoring the AIA results. Quadratic weighted kappa was 0.932 (95% CI: 0.800-1.000) for IBC and 0.706 (95% CI: 0.343-1.000) for SDC.
Conclusion: The automated method demonstrated good-to-excellent agreement with conventional assessment, particularly in breast tumors. Tissue matching tools may enhance HER2 amplification detection accuracy in heterogeneous tumors.
