Título: DIAGNOSTIC PERFORMANCE IN ASYNCHRONOUS TELEDENTISTRY FOR ORAL LESIONS: INSIGHTS FROM PRIMARY CARE

Resumo: Objective:
To evaluate diagnostic agreement and reclassification patterns between primary care professionals and specialist consultants in asynchronous teledentistry for oral lesion assessment.

Study Design:
Retrospective cross-sectional study including 600 teleconsultations submitted to a public teledentistry platform (2022-2025). Diagnostic hypotheses were categorized into benign lesions, potentially malignant disorders, and malignant neoplasms. Agreement was assessed using Cohen's kappa.

Results:
Among 479 cases, moderate agreement was observed (κ = 0.49), with overall concordance of 78.5%. Lesions initially classified as benign were upgraded in 12.3% of cases, while 41.7% of lesions suspected as malignant were reclassified into lower-risk categories. Potentially malignant disorders represented 23.0% and malignant neoplasms 18.7% of specialist diagnoses. Most cases were asymptomatic (67.3%), and patient-related factors showed no consistent association with diagnostic discrepancy.

Conclusion:
Asynchronous teledentistry demonstrated moderate diagnostic performance and clinically relevant reclassification patterns, supporting its role as a decision-support tool for oral lesion assessment and risk stratification in primary care.
