Título: MANAGEMENT OF MRONJ RISK IN PATIENTS LIVING WITH HIV/AIDS USING ORAL BISPHOSPHONATES: A CASE SERIES

Resumo: Alterations in bone metabolism are frequent in people living with HIV/Aids. The interaction between adverse effects of combined antiretroviral therapy (cART), osteoarticular aging, and persistent viral inflammation leads to reduced bone density and osteoporosis. Clinical management is challenging, as bisphosphonates increase the risk of medication-related osteonecrosis of the jaw (MRONJ) following invasive dental procedures. In this case series, seven patients (mean age 57.4 yrs) on cART and sodium alendronate (mean time use= 2.8 yrs) requiring tooth extraction were included. The protocol consisted of preoperative amoxicillin (500 mg TID), initiated 24 hours prior to surgery, atraumatic extraction, and photobiomodulation irradiation immediately postoperatively and on postoperative day 7. All cases, except one, achieved healing by primary intention with complete epithelialization within 14 days, without bone sequestration, persistent pain, or suppuration. These findings suggest that the protocol may offer a safe approach for preventing MRONJ in this high-risk population. FAPESP grant #2025/02873-9
