Título: ATYPICAL LANGERHANS CELL HISTIOCYTOSIS IN A PEDIATRIC PATIENT: CASE REPORT

Resumo:
A 1-year-old male presented with a 3-week history of swelling in the left hard palate. Clinically, the lesion was red, partially ulcerated, and bleeding, measuring approximately 3 cm, with no bone involvement on occlusal radiography. Based on the clinical hypotheses of Langerhans cell histiocytosis, Burkitt lymphoma, myofibroma, and rhabdomyosarcoma, an excisional biopsy was performed under general anesthesia. Histopathological examination revealed a neoplastic proliferation of epithelioid and round to oval cells with abundant eosinophilic cytoplasm. Tumor cells exhibited reniform nuclei and nuclear grooves resembling "coffee bean" morphology, along with cellular pleomorphism, atypical mitoses, and prominent nucleoli. A background of eosinophils and multinucleated giant cells was also observed. Immunohistochemically, the lesion was positive for S100, CD1a, CD207, CD68, and cyclin D1, and negative for myogenin, desmin, HHF35, chromogranin, smooth muscle actin, BRAF V600E, and ALK. These findings supported the diagnosis of Langerhans cell histiocytosis, and the patient was referred for medical management.
